Life Calling Foundation’s Notes to Humanity is a growing global archive of handwritten responses to one basic yet vital question: “What does it mean to preserve your humanity in the Digital Age?” Notes to Humanity is a call to action, inviting people from all walks of life to reflect on and express their individual insight in how to safeguard their humanity in an age of growing digital dependence. Notes to Humanity is a transformative global movement, a living testament to our shared commitment to preserve and nurture the essence of what makes us human. Throughout its tenure, Notes to Humanity will come to life in diverse and creative ways—immersive exhibitions, bespoke experiences, and collaborative partnerships designed to engage leaders across fields and inspire the broader public.

PARTICIPANT RELEASE AND WAIVER OF LIABILITY

I, the undersigned participant (“Participant”), voluntarily agree to participate in the Notes to Humanity campaign and immersive exhibit (“Exhibit”), an initiative of the Life Calling Foundation. This initiative collects handwritten responses to the question: “What does it mean to preserve your humanity in the Digital Age?” and may involve interactive, visual, audio, and/or experiential components.
By signing this Participant Release and Waiver of Liability (“Agreement”), I acknowledge and agree to the following terms.

ASSUMPTION OF RISK

• Voluntary Participation: I understand that my participation is voluntary and may involve inherent risks such as sensory overstimulation, moving or uneven surfaces, unexpected elements, or other unforeseen circumstances.
• Health and Safety: I confirm that I am physically, mentally, and medically fit to participate and will take all necessary precautions for my safety, including refraining from participation if I have medical conditions that could be exacerbated.
• Awareness of Risks: I assume full responsibility for any risks of bodily injury, emotional distress, property damage, or other harm arising from my participation.

RELEASE OF RIGHTS & LIABILITY WAIVER

Media Release: I grant Life Calling Foundation and its members, affiliates, successors, and assigns the irrevocable and unrestricted right to:

• Use, reproduce, modify, prepare derivative works from, distribute, and publish my likeness (photo/video), voice, written contributions, or other media captured during my participation (“Materials”).
• Utilize these materials in any form of media, now or in the future, for promotional, educational, or commercial purposes.
• I waive any right to compensation, approval, or future claims related to the use of my likeness or Materials, including claims of defamation, invasion of privacy, infringement of moral rights or intellectual property rights.

Liability Waiver & Indemnification:

• I release and forever discharge Life Calling Foundation, its officers, employees, agents, contractors, and affiliates (“Released Parties”) from all claims, liabilities, demands, or causes of action arising out of my participation, including personal injury, death, property damage, or emotional distress.
• I waive any claims related to negligence or unforeseen incidents that may arise.
• I agree to indemnify and hold harmless the Released Parties from any claims or liabilities arising from my acts, omissions, or breach of this Agreement.

ACKNOWLEDGMENT AND AGREEMENT

I have carefully read and understand this legally binding Agreement. I acknowledge that by signing below, I:

• Voluntarily accept all risks and responsibilities associated with participation.
• Understand that I am waiving certain legal rights.
• Agree that this Agreement is binding upon myself, my heirs, legal representatives, and assigns.
